Chronic Cough
A cough that persists is a symptom, not a diagnosis. Allergy, asthma, chronic rhinitis or sinus disease, reflux, medication effects, infection, and other lung or airway conditions can create similar patterns.

Details that narrow the cause
Note when the cough began, whether it is dry or productive, what happens at night or with exercise, and whether there is wheeze, nasal drainage, heartburn, voice change, fever, or weight loss. Include smoking, vaping, workplace exposure, and medication history.
Prior chest imaging, lung testing, inhaler trials, and treatment response are often more useful than repeating a broad panel.
Possible testing
The next step depends on the pattern. Spirometry, bronchodilator testing, targeted allergy evaluation, FeNO, imaging review, or referral to another specialty may be considered. No single test explains every chronic cough.
A normal test can still be useful because it changes which causes remain likely and prevents unnecessary treatment escalation.

Seek prompt care for coughing blood, severe breathing difficulty, blue or gray lips, chest pain, fainting, confusion, or a rapidly worsening condition. A routine allergy appointment is not emergency care.
For a stable persistent cough, bring a symptom timeline and current medicines so the visit can focus on the most likely mechanism.

Upper-airway inflammation, asthma, reflux, medication effects, and environmental irritation can coexist. Improvement with one treatment does not always prove that it was the only cause, and failure of a short trial may reflect technique, duration, adherence, or an incorrect working diagnosis.
A structured timeline helps the clinician compare cough with nasal symptoms, meals, sleep, exercise, infections, travel, and medicine changes instead of treating every possible cause at once.
What a focused cough workup avoids
Broad allergy panels, repeated antibiotics, and multiple inhalers can add confusion when they are not tied to objective evidence. A focused evaluation selects the next test or trial based on the most likely mechanism and defines what improvement would count as a meaningful response.
Bring the actual inhalers, a complete medication list, prior imaging reports, and test tracings when available. The original report often contains details that are missing from a portal summary.
Know the Next Step
Questions to answer before follow-up
Track cough frequency, nighttime waking, exercise tolerance, sputum, wheeze, throat clearing, nasal drainage, heartburn, rescue-medicine use, and any days completely free of symptoms. Note whether the cough changes away from home or work.
At follow-up, ask which diagnosis is currently most likely, what evidence supports it, how long the treatment trial should continue, and what result would trigger imaging, pulmonary review, ENT evaluation, or another next step.
Separate Overlapping Causes
A cough timeline is more useful than repeating unrelated tests
Describe the cough by pattern: dry or productive, daytime or nighttime, sudden or gradual, and triggered by exercise, meals, talking, cold air, fragrance, or lying down. Note wheeze, throat clearing, voice change, nasal symptoms, heartburn, fever, weight change, and breathlessness. These details help prioritize airway, upper-nasal, reflux, medication, and other explanations.
Bring the names and duration of previous inhalers, nasal sprays, reflux treatments, and antibiotics. A short or inconsistent trial may not answer the intended question, while repeated treatment without an objective endpoint can obscure the pattern. Prior imaging, spirometry, and laboratory reports should be reviewed rather than summarized only as “normal.”
More than one mechanism may be active. The plan should define which cause is being tested, how long the trial will run, and what result would support continuing, changing, or stopping it.

Common questions during a chronic cough evaluation
Can asthma cause cough without obvious wheezing?
Yes. Some patients have cough-predominant asthma, but cough alone does not establish the diagnosis. History, lung function, treatment response, and sometimes additional airway testing are interpreted together.
Does a normal chest image explain the cough?
A normal image can reduce concern for some structural problems, but it does not identify every upper-airway, asthma, reflux, medication, or laryngeal cause. The next step still depends on the clinical pattern.
