Peanut allergy is a common cause of food-related anaphylaxis. Accurate diagnosis matters because sensitization on a test does not always mean a person will react when peanut is eaten.

Review labels, restaurant cross-contact, school or workplace plans, and prescribed epinephrine. Families should know when to treat a reaction and should not rely on antihistamines for anaphylaxis.

Peanut is a legume, but co-allergy is individual
Peanut is botanically related to beans, peas, lentils, and soy rather than tree nuts. Most people with peanut allergy do not need to avoid every legume, and a peanut diagnosis does not automatically prove tree-nut allergy. Foods already eaten without symptoms should not be removed solely because of botanical relationships.
Targeted evaluation can address any separate food that has caused symptoms or has never been introduced.
Everyday exposure planning
Read labels each time and ask restaurants about sauces, desserts, shared equipment, and cross-contact. Schools and caregivers need a written emergency plan, accessible prescribed epinephrine, and clear authority to act. Adolescents should have a transition plan for carrying medication and communicating risk independently.
Skin contact or odor alone does not represent the same exposure as ingestion, but residue can move from hands and surfaces to the mouth, so practical cleaning still matters.

Treatment goals must be explicit
Peanut oral immunotherapy and biologic treatment may be discussed for selected patients. These approaches aim to reduce risk from accidental exposure; they do not guarantee unrestricted eating, remove all reaction risk, or eliminate the need for emergency readiness.
Compare expected benefit with dosing burden, reactions, exercise and illness precautions, visit frequency, cost, and the need for continued maintenance.
Match Testing to Actual Risk
Peanut test numbers support probability, not a stand-alone diagnosis
A peanut evaluation begins with the food form, amount, timing, symptoms, treatment, and any peanut exposure tolerated before or after the event. Skin-prick, peanut-specific IgE, and selected component testing can refine probability, but no single cutoff predicts exactly what will happen during a future exposure.
When history and testing do not agree, a supervised oral food challenge may be discussed for an appropriate patient. Challenges are structured diagnostic procedures with measured doses, observation, and treatment available; they are not suitable for home experimentation after a concerning reaction.
Management should cover ingredient labels, bakery and restaurant cross-contact, school or childcare procedures, travel, epinephrine technique when prescribed, and whether oral immunotherapy fits the patient’s goals. OIT can raise the reaction threshold for selected patients, but it does not remove daily dosing responsibilities or all exposure risk.

Is peanut a tree nut?
No. Peanut is a legume. Peanut and tree nut allergies are separate diagnoses, although a person can have both and cross-contact can affect practical avoidance.
Can a component test predict reaction severity?
Component testing may improve probability assessment in selected cases, but it cannot reliably predict the exact severity of a future reaction. Emergency preparedness remains important.
Does peanut OIT cure the allergy?
No. OIT is a risk-reduction treatment intended to increase the amount that triggers a reaction for selected patients. Ongoing dosing, precautions, follow-up, and emergency readiness are still required.
Should a peanut test be repeated every year?
Retesting intervals depend on age, reaction history, prior values, and whether the result could change a challenge or treatment decision. Repeating a number without a clinical question may not alter the plan.
