Insect Sting Allergy

Record the insect if known, setting, number of stings, time to symptoms, body systems involved, treatment, and emergency records. Photographs can help identify the culprit but are not always conclusive.

For a compatible systemic reaction, an allergist may use venom skin testing, blood testing, or both. Timing after the event and the reaction history affect interpretation.

For appropriate patients with a systemic reaction and supporting evaluation, venom immunotherapy can substantially reduce the likelihood of another systemic reaction while treatment continues. The decision considers the reaction, test results, exposure risk, other health conditions, medicines, and ability to complete treatment.

Reduce exposure without giving up outdoor life
Wear closed shoes outdoors, use caution around open drinks and food, keep waste covered, and arrange professional removal of nests near occupied areas. Fragrances and bright clothing may attract some insects, but no avoidance strategy removes all risk.
Carry prescribed epinephrine, keep devices within temperature limits, and make sure family or coworkers know the emergency plan when exposure is likely.
Separate Local Swelling from Systemic Risk
The reaction pattern determines whether venom testing can change care
Pain, redness, and swelling around a sting are common. A large local reaction can extend beyond the sting site and last several days, but it is different from a systemic reaction involving hives away from the sting, breathing difficulty, throat symptoms, dizziness, low blood pressure, or widespread gastrointestinal symptoms.
For a systemic reaction, identify the likely insect, setting, season, sting location, symptom timing, emergency treatment, and prior tolerated or reactive stings. Venom skin and blood testing are interpreted with that history. Testing people who have only expected local reactions may produce sensitization results that do not establish a dangerous clinical allergy.
Venom immunotherapy may markedly reduce systemic sting risk for appropriately selected patients. The plan also includes avoidance around nests and outdoor food, epinephrine access when prescribed, and instructions for what to do after a future sting.

Is every large swollen sting anaphylaxis?
No. Large local reactions can be extensive but remain centered around the sting site. Symptoms in distant skin, breathing, circulation, or multiple body systems suggest a systemic reaction and require a different assessment.
Are mosquito reactions evaluated with venom testing?
No. Standard venom allergy testing and immunotherapy focus on stinging insects such as bees, wasps, hornets, yellow jackets, and, in relevant regions, fire ants. Mosquito reactions follow a different pathway.
Do I still need epinephrine during venom immunotherapy?
Follow the prescribing allergist’s plan. Device recommendations depend on reaction history, treatment stage, exposure, and individual risk; immunotherapy appointments do not replace an emergency action plan.
