Shellfish Allergy
Shellfish includes crustaceans such as shrimp, crab, and lobster and mollusks such as clam, scallop, oyster, and squid. Allergy to one group does not automatically define tolerance to every species.

Separate shellfish from fish
Finned fish and shellfish contain different major allergens. Document the exact species, preparation, restaurant setting, amount, timing, and whether other seafood has been tolerated.
Testing and cross-reactivity
Targeted skin or blood testing may help assess crustaceans and selected mollusks. Cross-reactivity is common within crustaceans, but results do not replace the clinical history or automatically prove allergy to every shellfish.

Shrimp, crab, and lobster are crustaceans; clams, oysters, scallops, mussels, squid, and octopus are mollusks. Allergy within one group does not automatically establish allergy to every species in both groups, although cross-reactivity can occur.
Record the exact species and preparation instead of using “seafood” as the diagnosis. Fish belong to a separate biological group and require their own history.
Shellfish allergy is not an iodine allergy
Shellfish proteins cause shellfish allergy. Iodine is not the allergen, and a shellfish history by itself does not predict a reaction to iodinated contrast material. Contrast reactions require their own risk assessment based on the actual prior contrast exposure and other clinical factors.
Keeping the chart label precise prevents unnecessary avoidance and helps radiology teams ask the right safety questions.

Restaurant exposure can occur without ordering shellfish
Shared fryers, grills, woks, steam, utensils, sauces, stocks, and preparation surfaces can create exposure. Seafood restaurants may be difficult environments for highly sensitive patients even when a non-shellfish dish is ordered.
Ask specific questions before ordering and leave when the preparation cannot be clarified. Follow the prescribed emergency plan and do not use a restaurant meal to test whether another shellfish is tolerated.
Separate Crustaceans from Mollusks
Shellfish testing should identify the exact group and species in question
Crustaceans include shrimp, crab, and lobster; mollusks include scallops, clams, oysters, mussels, squid, and related species. Cross-reactivity is common within groups but is not absolute. Record the exact species, preparation, amount, timing, and whether other shellfish have been eaten safely.
Tropomyosin is a major cross-reactive protein also found in dust mites and cockroaches. Sensitization from inhalant allergy can complicate shellfish test interpretation, particularly when there has never been a reaction. Results should therefore answer a defined clinical question rather than create a blanket diagnosis from a panel.
Restaurant grills, fryers, seafood steam, and mixed dishes create practical cross-contact concerns. The final plan should distinguish shellfish from finned fish and address emergency medication, label reading, and whether selected supervised challenges could safely clarify the diet.

Shellfish cross-reactivity and safety questions
Is shellfish allergy related to iodine?
No. Iodine is not the allergenic protein in shellfish. A shellfish allergy does not itself mean a patient is allergic to iodinated contrast or povidone-iodine.
Does shrimp allergy prove allergy to every mollusk?
No. Crustaceans and mollusks are different groups. Cross-reactivity patterns and individual tolerance vary, so any testing or challenge should focus on named species.
Can shellfish allergy begin in adulthood?
Yes. Shellfish allergy can first appear in adults. A new reaction still needs evaluation because toxins, contamination, and other ingredients can mimic or complicate the history.
Does cooking remove shellfish allergens?
Not reliably. Major shellfish proteins can remain allergenic after boiling, grilling, or frying, and cooking steam or shared surfaces can matter for some highly sensitive patients. Follow the species-specific avoidance and restaurant plan provided after evaluation. A cooked shellfish should never be used as an informal home test after a systemic reaction, even when another preparation seemed less reactive. Preparation does not establish tolerance to that species.
